This Company will use the personal information subject to disclosure that it collects only for the objectives for use described in “2. Objectives for the Use of Personal Information Subject to Disclosure”.
Except in cases covered by law etc., we will not disclose or provide personal information to third parties.

5. Entrustment of Personal Information to Third Parties

This Company may entrust only the personal information subject to disclosure of users of this Company’s services to a subcontracted company to carry out necessary operations such as the provision of products and services or the billing of payments for products or services. In such a case, the Company shall select only companies that fulfill standards established by this Company, impose contracts pertaining to the handling of personal information, and conduct appropriate supervision.

6. Procedures to Request the Disclosure, Correction, Cease of Use,
Etc. of Personal Information

If an individual, their legal representative, or other authorized representative requests the notification of objectives for use of personal information, the disclosure of personal information, the correction of its contents, amendments to, removals from, deletions of, or the cease of use of personal information (hereafter, “Procedure to Request Disclosure Etc.”, the Company shall respond according to the following procedures to an application form established by this company.

If you wish to file a claim in accordance with Procedures to Request Disclosure Etc. for your own personal information, please contact us through the methods listed under “9. Contact Information for Complaints, Consultations, and Inquiries”. Once you have done so, this Company will send an Application for Disclosure Etc., which you will need to complete.

Please complete the Application for Disclosure Etc. that you will have received according to 1. above and submit documents with which you can verify your identity as described in (1) through (3) below via “9. Contact Information for Complaints, Consultations, and Inquiries” by mail. If you are submitting these procedures as a representative for another individual, a power of attorney is needed from that individual as well.

(1) Documents for Identity Verification for Procedures to Request Disclosure Etc. by an Individual

One document from [A] below and one document from [B] below

  • A copy of your seal registration document (for documents within 3 months of their effective date, must
    have current address)
  • A copy of the official family register (must be within 3 months of effective date)
  • A copy of your certificate of residence (must be within 3 months of effective date)

【B】

  • A copy of your passport (must be currently valid and have your current address)
  • A copy of your driver’s license (must be currently valid and issued by the public safety division of an
    administrative area of Japan)
  • A copy of a valid official document such as an insurance card by which your identity can be verified
    (must be currently valid)

(2) Documents for Identity Verification for Procedures to Request Disclosure Etc. by a Legal Representative

One document from [A] below and one document from [B] below.
【A】

  • A copy of your seal registration document (for documents within 3 months of their effective date, must
    have current address)
  • A copy of the official family register (must be within 3 months of effective date)
  • A copy of your certificate of residence (must be within 3 months of effective date)
* Limited to documents by which relationship with the individual can be confirmed.

【B】

  • A copy of your passport (must be currently valid and have your current address)
  • A copy of your driver’s license (must be currently valid and issued by the public safety division of an
    administrative area of Japan)
  • A copy of a valid official document such as an insurance card by which your identity can be verified
    (must be currently valid)

(3) Documents for Identity Verification for Procedures to Request Disclosure Etc. by an Authorized Representative

The Power of Attorney which indicates the representative as well as one document from [A] below and one document from [B] below (for both the individual as well as the representative)

  • A copy of your seal registration document (for documents within 3 months of their effective date, must
    have current address)
  • A copy of the official family register (must be within 3 months of effective date)
  • A copy of your certificate of residence (must be within 3 months of effective date)

【B】

  • A copy of your passport (must be currently valid and have your current address)
  • A copy of your driver’s license (must be currently valid and issued by the public safety division of an
    administrative area of Japan)
  • A copy of a valid official document such as an insurance card by which your identity can be verified
    (must be currently valid)

(4) Cautions Regarding Documents for Identity Verification

  • Please send documents with portions pertaining to your current domicile carefully and completely
    blacked out.
  • If the required facts are on the reverse side, a copy of the reverse is also required.

(5) Fees

All costs incurred by these procedures (such as the fees for having official documents issued or copying expenses) as well as any postage fees incurred when sending documents to this Company are the responsibility of the user. This Company shall pay any fees incurred when sending documents to the user.

(6) Non-Disclosable Information

When this Company receives a Request to disclose personal information subject to disclosure, we will conduct a review of the contents and notify the requestor of the results at a later date.
However, under the circumstances described below, this company may treat personal information as non-disclosable.

  • If we are unable to verify the individual’s identity.
  • In the case of a request made by a legal representative, if we are unable to verify the relationship
    between the requestor and the individual.
  • In the case of a request made by a representative, if we are unable to verify the right of the requestor to
    represent the individual.
  • If there is any defect in the submitted documents.
  • If the items requested according to the disclosure request procedure do not constitute personal
    information subject to disclosure.
  • If there is a risk of harm to the life, safety, property, or other rights or interests of the individual or any
    third-party.
  • If there is a risk of imposing a considerable hindrance to the operations of this company
  • If disclosure would constitute a violation of any other law.

(7) The Objectives for the Use of Personal Information Collected Pertaining to Applications

The personal information provided pertaining to requests for procedures to disclose information will be handled only as necessary for the investigation pertaining to the procedures, to confirm the individual’s identity, to respond to the request, and for other related activities. Additionally, once we have responded to your request, all relevant documents will be promptly disposed of and we will be unable to respond to requests for their return.
